Intake Forms
After scheduling your initial appointment, the next step in
preparing for counseling is to complete the appropriate intake forms.
Links to each are provided below, along with an explanation of which forms you
need to fill out. Please arrive 15 minutes prior to your first appointment with
your paperwork completed. If you have any questions regarding which set of
intake forms to complete or if you have any other questions regarding your
appointment, please call our office at (919) 872-1818. Downloading these forms
requires Adobe Reader which can be downloaded for free.
COUNSELING POLICIES FORM
All adult clients or family units must read, initial and sign this three page form.
ADULT INTAKE FORM
Adult clients (18 years and older) will need to complete all aspects of this form. If you are coming as a couple for marital counseling, you and your spouse may complete one form as a couple. You will also need to complete a Counseling Policies Form.
PARENT INTAKE FORM and MINOR INTAKE FORM
Any minor (under 18 years of age) who will be receiving counseling at CrossLife Ministries will need to have both of these forms completed. Both parents will need to read and sign a Counseling Policies Form for the family. Parents/legal guardians must attend the initial interview appointment.
PRE-MARITAL INTAKE FORM
If you are coming for pre-marital counseling, you and your fiancé will each need to complete this form and a Counseling Policies Form independently.
